Upon discharge, the referring physician should receive a faxed Discharge Planning Sheet, which contains the following information:
- Diagnosis
- Procedures performed
- Discharge instructions, i.e., medications and follow-up appointment
- Name of Home Health agency
- Name of durable medical equipment company
