The camp application process is very important. The medical staff needs to have a lot of information to enable us to make appropriate decisions about your child’s ability to attend camp. In order to save as many trees as possible, and be true to our past "green" theme, please print only the pages needed for application. If you print them on front and back you will save even more paper.

You must have Adobe Acrobat Reader installed in your computer to view the files.

Important!! Please print, complete, and return the Camp EB Application Packet only!
