Planificación del Alta
Los pacientes se evalúan en el momento de la admisión para posible planificación de medios de alta y asistencia.Las referencias de planificación del alta pueden iniciarse a través de una orden sometida en el formulario de Ordenes Médicas. Al recibir la referencia, el administrador de casos realizara una evaluación y comunicara el plan en las notas de progreso.

Comunicación del Médico Referente
Al momento del alta, el médico referente deberá recibir un Resumen Post-visita vía fax que contenga la siguiente información:- Diagnóstico
- Procedimientos realizados
- Instrucciones del alta, tal como medicamentos y citas de seguimiento
- Nombre de la agencia de salud del hogar
- Nombre de una compañía de equipo médico de confianza
