Which vaccine will I receive?

Right now there are three types: Pfizer, Moderna and Johnson & Johnson (J&J)/Janssen. We do not know which one we will get until it has been received.

How do I sign up for the second (2) dose?

We will automatically sign you up for the second dose on the day you receive

your first dose. The date of your second dose will depend on which vaccine you

received.

What if I need to cancel my appointment or I

entered my information incorrectly?

Contact the Call Center at 1-844-977-1477 or email vaccine@dchstx.org.

Should I consent to have Texas Immtrac keep

the record of my vaccination for COVID-19?

Yes, the Immtrac record can be accessed in the future to prove you are

vaccinated. This may prove to be helpful in determining employability for certain

healthcare positions just as proof of immunity to common childhood diseases is

currently required to work in the children’s hospital or it may be required in

the future for travel.
