Referrals
A referral is not required of Driscoll Health Plan members to begin receiving treatment from Driscoll’s Child and Adolescent Psychiatry Clinic. A New Patient Intake Packet needs to be completed and turned in before booking a new patient appointment.

First Visit
Each new patient is asked to complete a New Patient Intake Packet, featuring health history, and a Vanderbilt Assessment form before the first visit. The packet must be submitted prior to scheduling the first appointment. This will help our medical staff make a prompt and accurate assessment during the first visit. Please bring your current insurance cards, including Medicaid cards, and any current prescription bottles. You will need to arrive on time. Patients who arrive 15 minutes late may have to be rescheduled.

Your child will accompany you for the evaluation. If you prefer to speak with the doctor alone, please bring someone to watch your child in the lobby. First appointments can last between 60 and 90 minutes.

Prior to all sessions, the patient is prepared and triaged with a nurse or medical assistant. Vital signs will be taken, and all relevant information will be noted, including current medication.

Routine or Follow-Up Visit
Always bring your insurance cards, including Medicaid cards, and all prescription medications to your appointments.

Cancellation Policy
Please call at least 24 hours in advance to cancel any appointments, so that we may offer the time slots to other patients in need of treatment. We do our best to see every patient at their scheduled time; however, we reserve the right to reschedule your appointment in the event of a provider emergency or if the patient arrives 15 or more minutes late for an appointment. Same-day cancellations are counted as “no-shows.” Excessive no-shows will lead to possible discharge from the clinic.

Patient Forms
The following forms are available for download. Fill out and bring the appropriate forms to your appointment as requested.

Billing and Insurance
The Child and Adolescent Psychiatry Clinic is available to Driscoll Health Plan members and accepts BCBS of Texas. Starting 2/1/2021, Humana Commercial Insurance will also be accepted.

Prescriptions and Refills
Please bring all prescription medications, including vitamins and herbal supplements, to each appointment, even if prescribed by another physician. Whenever possible, please ask for all refills of continuous medications during your appointment.

If your prescription expires before your next visit, please contact our office for refills. Please allow 7–10 business days to process refill requests. All controlled substance prescriptions must be picked up in our office and all other non-controlled substance prescriptions will be faxed to your pharmacy. Please provide us with your current pharmacy information.

Transportation Benefits via Medicaid
Children with Medicaid, along with one parent, may request transportation to and from appointments at no charge. This benefit also covers the cost of transportation when you need help getting to and from appointments. This includes the cost of a bus ride or ride-sharing services.

A Family and Friends reimbursement option is also included. Anyone who drives you to a scheduled medical visit can be paid per mile driven. Drivers must have a current driver’s license, inspection sticker, license plate, car insurance and Social Security number.

For more information, call 1-877-MED-TRIP (1-877-633-8747), Monday through Friday, 8 a.m. to 5 p.m., or visit the Medical Transportation Program online.

Medical Transportation Program

Please use the Physician Referral Form. Note: Psychotherapy is not provided at this time. If psychoeducational testing has already been completed, please send the results.
